DeArmond 610 Volume Tone Pedal trouble

Folks,
Recently I'm having trouble with the volume gear detaching from the pot gear... see videos below. I opened it up a while back trying to see if I could ensure the tone pot was rolling all the way back in order to get more tone rolled off, and since then, I've had this problem with the volume gear. I'm just about certain I didn't loose any parts when reassembling, but I may have. Anyone have suggestions to remedy this?

Here's the fix, a nylon washer and a lock washer. See video, let me know if you can't see the vid. Wish I had a taller nylon washer, but this should do the trick for my use.
